By submitting your allocation of proxy for the AGM on Wednesday 25 June 2025, please note the following:
- You are allocating a proxy as you are not able to attend the AGM; and
- Completing this form is the only proxy permitted.
If you decide to attend in person or online after you have allocated a proxy, please email election@amawa.com.au to advise that you are cancelling your proxy.
